弗洛伊德认为强迫症的原因是：强迫症是病理的强迫性人格的进一步发展，是由于防御机制不能处理强迫性人格而形成的焦虑，于是产生了强迫性症状。当本我的情欲与攻击驱力和超我要控制和调节这类驱力发生冲突时，自我如果不能运用理性机制来调节它们的冲突以及缓解冲突引起的焦虑，就不得不采用一些来避免焦虑。假如这些防御机制不足以抵御或缓解这些焦虑时，为了替代本能欲望冲动，强迫症性冲突就产生了。由于本我要寻求的表现的本能冲动处于潜意识领域，自我就很难意识到其冲突的真正对象，因此就体验到莫名的恐惧、焦虑。当焦虑转换为躯体症状时，则表现为癔症的转换性症状；焦虑被分离出意识时，则表现为癔症的分离性症状；当焦虑被转向外部世界的对象时，则表现为恐惧症；当焦虑被隔离开时，则表现为强迫症；如果焦虑被直接体验，则表现为焦虑。这些症状可理解为“妥协形成”或自我企图来整合自我驱力、超我与现实的结果。
