Congenital heart disease (CHD) is the most common birth defect worldwide. There are now more than 1,000,000 adults in the United States living with CHD. More than half of patients over 18 years of age are lost to appropriate follow up. A lapse in care increases the risk of complications dramatically Follow My Heart was designed as a team effort between the Division of Cardiology/Washington Adult Congenital Heart Program at Children’s National Medical Center and the Washington Hospital Center, Washington, DC and FollowMe. www.healthshack.info is a Personal Health Information System (PHIS) developed for homeless, system based and emancipating foster youth. Launched in January 2010, HealthShack is a youth controlled and youth promoted information system that allows members to store health information and includes additional portals for storing education history, work history, and other important life documents. HealthShack was developed in partnership with WIND Youth Services in Sacramento, California and UC Davis Dept. of Adolescent Medicine and FollowMe. HealthShack licenses and memberships are available to organizations serving vulnerable youth. Launched in 2003, MyHIN provides peace of mind to hundreds of families and individuals who need quick access to their medical information. www.healthbio.org is a community care record developed for a health partnership serving people with developmental disabilities based in Deluth, MN.
